Participants Consent

   I hereby voluntarily give consent

to Meira (Minna) Herskowitz  to engage in a physical exercise program. I recognize that exercise carries some risk to the musculoskeletal system (sprains, strains, etc.), circulatory system (abnormal blood pressure, fainting, disorders of heart beat, and very rare instances of heart attack), as well as other systems in the body.  I understand that every effort will be made to minimize problems by preliminary examination and observation during exercise. I also discussed any injuries I may have with my doctor and got his/her permission to engage in an exercise program. 

I understand that I am responsible for monitoring my own condition throughout any exercise. Should any unusual symptoms occur, I will cease my participation and inform my personal trainer of my symptoms. Also, in consideration of being allowed to participate in a fitness program, I agree to assume all risks of such fitness exercise. During the training session if I feel pain or any other symptoms of an illness or injury, I will notify my personal trainer and the exercise will cease.

 

  • I have been advised to consult a physician prior to beginning this program.
  • I have been advised to cease exercise immediately if I experience any unusual discomfort and feel the need to stop.

 

I have read and understand the above agreement. I have been made fully aware of and understand the potential risks involved in this physical fitness program. I am freely and voluntarily participating in the exercise program given to me and understand all risks involved. I am freely and voluntarily signing this agreement.
